The facility failed to properly supervise Alleged Victim (AV) which resulted in AV eloping from the facility on November 24, 2020 and again on November 27, 2020. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
