Findings
The facility failed to provide Alleged Victim (AV) basic care, services, and safety which resulted in AV suffering multiple falls with injury. An investigation determined that facility neglected AV by not implementing interventions to address AV's fall risk. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
