Findings
The facility failed to provide a safe environment for Alleged Victim (AV) by failing to implement reasonable interventions aimed at preventing or mitigating AV's frequent falls which resulted in AV having nine (9) falls between September 2, 2020 and November 24, 2020. AV suffered multiple injuries from these falls including a dislocated shoulder. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
