Findings
The facility failed to provide a safe environment for Alleged Victim (AV) which resulted in AV suffering six (6) non-injury falls between July 17, 2020 and September 13, 2020. The facility's failure to implement and follow appropriate interventions for AV's fall risk is a violation of resident rights is considered neglect care and constitutes abuse.
