Findings
The facility failed to provide adequate supervision of Alleged Victim (AV) which resulted in AV being kicked in his/her head multiple times and received a scratch on his/her left temple and left finger. The facility's failure to protect AV from Witness 1 (W1) is a violation of resident rights, is considered neglect of care and constitutes abuse.
