Findings
The facility failed to provide appropriate services according to the Alleged Victim’s (AV) needs for mobility and incontinence care. The failure resulted experiencing several falls and lack of necessary assistance resulting in unreasonable discomfort and a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
