Findings
On or about November 29, 2020, Alleged Perpetrator 2 (AP2) yelled at Alleged Victim (AV) while AP2 was assisting AV with his/her Activities of Daily Living. AP2's actions is considered verbal/emotional abuse. The facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
