Findings
Witness #1 was known to have aggressive behaviors the months prior to this altercation. On or about December 1, 2020, the Alleged Victim (AV) and Witness #1 (W1) engaged in a physical altercation. AV touched W1's foot and W1 swung at AV and missed but then hit AV in the left jaw. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
