Findings
On or about November 3, 2020, Alleged Victim (AV) was found to have passed away. Staff prepared AV and recall AV was wearing his/her four (4) rings. The funeral home picked up AV and did an inventory of his/her belongings that did not include the rings. It is believed the rings were taken by Alleged Perpetrator 2 (AP2), an unknown staff member. The facility does not have a procedure for doing an inventory or securing a resident's valuable after death. The facility failed to protect AV from financial exploitation, which is a violation of resident rights, is neglect of care and constitutes abuse.
