Findings
On or about December 2, 2020, Alleged Perpetrator 2 (AP2) pushed the Alleged Victim's arm down in a forceful manner during care. AP2’s actions are considered a violation of resident rights, is considered neglect of care and constitutes physical abuse. The facility failed to provide a safe environment which violates Oregon Administrative Rules.
