Findings
On or about December 6, 2020, the Alleged Victim (AV) and Witness #1 (W1) had a non-injury altercation. The facility failed to provide a safe environment for the AV, resulting in AV being slapped by W1 and W1. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
