Findings
Alleged Victim (AV) relies on facility staff for incontinence care. On or about December 4, 2020, AV used his/her call light to get assistance with incontinence care after a bowel movement. AV reports that Alleged Perpetrator 2 (AP2) refused to assist him/her. Call light records indicate AV's call light was not answered timely. AP2's actions are considered neglect of care and constitutes abuse. The facility failed to ensure AV's care plan was followed which is a violation of Oregon Administrative Rules.
