Findings
On or about December 15, 2020, the Alleged Victim (AV) and Witness #1 (W1) were sitting next to each other and engaged in an altercation where W1 hit AV in the face with a TV Tray. AV and W1 have had previous altercations and there was a plan in place for them to not sit next to each other. There was not enough staff on shift to meet the scheduled and unscheduled needs of residents. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
