- Alleged Victim (AV) relies on the facility for his/her care. From August 23, through December 13, 2020, AV had at least seventeen (17) falls and out of those, AV had six (6) falls with injuries. On or about October 23, 2020, a fall was recorded as a non-injury fall, but AV was taken to the emergency room due to a hip discoloration; this was not documented until AV was taken off alert charting from the fall. The facility failed to properly plan care around AV’s known fall history, resulting in repeated unreasonable discomfort and injury, which is a violation of resident rights, is neglect of care and constitutes abuse.
