Findings
The facility failed to provide Alleged Victim (AV) the basic care and services needed to ensure AV had adequate meal and fluid intake which resulted in AV sustaining undesirable weight loss and a decline in overall health. AV was sent to the hospital for treatment. The facility's failure to properly plan care for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
