Findings
The facility failed to provide appropriate care and oversight over Witness 1 according to his/her known agitation and recent altercation. The failure resulted in W1 being questioned about another altercation when he grabbed the Alleged Victim and causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
