Findings
On or about December 19, 2020, Alleged Victim (AV) had a resident-to-resident altercation with Witness 1 (W1) that resulted in AV getting a bruise after W1 gripped AV's right arm. The facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
