The facility failed to appropriately monitor Witness 6 according to his/her known behaviors, agitation and prior altercations. The failure resulted in a physical altercation with the Alleged Victim, causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
