Findings: The facility failed to provide appropriate services and respond to the Alleged Victim’s (AV) pain following an unwitnessed fall. The failure resulted in AV experiencing extreme pain for approximately 15 hours before being sent to the hospital and diagnosed with a rib fracture, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
