Findings
On or about December 31, 2020, the Alleged Victim (AV) suffered an injury fall when he/she grabbed onto a rolling cart that was left unattended in the hallway without the wheels locked. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
