Findings
Alleged Victims (AV) relies on the facility for his/her care. On or about May 29, 2020, Alleged Perpetrator 2 (AP2) was assisting AV but was not explaining what he/she was doing. AP2 picked AV up and dropped AV on the bed. AP2 told AV he/she was acting like a two-year-old. AP2 actions are consider physical abuse. The facility failed to protect AV from physical abuse, which is a violation of Oregon Administrative Rules.
