Findings
On or about December 26, 2020, the Alleged Victim (AV) had several charges of approximately $500.00 made on his/her debit card. Upon looking, AV's debit card was missing and he/she had no idea where the card was. The card had been locked in AV's personal lock box and no one but AV had the key. Alleged Perpetrator #2's (AP2, an unknown individual) actions are considered theft and constitutes abuse by means of financial exploitation. The facility failed to protect AV from financial exploitation, which is a violation of Oregon Administrative Rules.
