Findings: The Alleged Victim (AV) was a known exit seeker and elopement risk. On or about January 1, 2021, AV was angry and exit seeking and broke down a fence in the patio area and left the facility. AV was later found unharmed. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
