Findings
Alleged Victim (AV) developed pressure ulcers due to lack of care by the facility which included inconsistent repositioning/offsetting pressure. As a result, AV was sent to the hospital for treatment. The facility's failure to properly plan care for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
