Findings
The facility failed to provide appropriate services according to the Alleged Victim's (AV) needs after he/she experienced a fall with injury. The failure resulted in AV not receiving staff assistance and had to put him/herself back to be alone and in pain, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
