The facility failed to follow doctor's orders to turn/change AV's position, and providing appropriate hygiene care. The failure resulted in AV experiencing unreasonable discomfort, a loss of personal dignity, and placed at risk of serious harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
