Findings
Alleged Victim (AV) is a known elopement risk. On or about January 3, 2021, AV was found on the independent living side of the building by another resident and escorted back to the memory care unit. It is unknown how AV eloped from the locked area of the building. The facility did have two staff working on the unit that were borrowed from a sister facility. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
