Findings
The facility failed to ensure the safety of Alleged Victim's (AV) mobility which resulted in AV suffering seven (7) falls between November 6, 2020 and January 3, 2021, with five (5) of the seven (7) falls having AV being sent to the hospital for treatment. The facility's failure to properly care plan for AV's fall risk is a violation of resident rights, is considered neglect of care and constitutes abuse.
