Findings
Alleged Victim (AV) is a known fall risk and is care planned to have an alarm tab while in bed. On or about January 7, 2021, AV was found on the floor with multiple skin tears. AV's tab alarm was not connected to AV causing the alarm not to alert staff. The facility failed to follow AV's care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
