Findings
On or about January 8, 2021, the Alleged Victim (AV) was found on the floor with a scratch on his/her back. AV was care planned for one hour safety checks and to have non-skid socks on. AV had not been checked every hour and was found without his/her non-skid socks on. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
