- On or about January 12, 2021, it was discovered that Alleged Perpetrator #2 (AP2) was removing narcotics from the med room, performing audits and destroying medications on his/her own without another staff member present to witness the destruction of medications. The Alleged Victim (AV) had two narcotic medications, one was discontinued and marked as destroyed by AP2 without verification of destruction. AP2 also marked destroyed a different narcotic belonging to AV without explanation of why it was destroyed and without secondary verification. AP2's actions are a violation of resident rights, is considered neglect of care and constitutes financial abuse. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
