Findings
The Alleged Victim (AV) moved into the facility on or about December 26, 2020, and was noted to be independent with transfers and toileting. Progress notes dated December 26, 2020 and December 28, 2020 note that AV requires assistance with transfers. AV had been observed to be unable to ease his/her self on to the toilet or into his/her wheel chair, resulting in AV "plopping" down onto the seats. On or about January 14, 2021, AV was observed to have bruising on his/her bottom, which through the facility investigation was from AV "plopping" his/her self down on hard plastic seats. The facility failed to care plan related to AV's needs with transfers, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
