Findings
Alleged Victim (AV) requires staff assistance with transfers and motility. On or about January 10, 2021, Alleged Perpetrator 2 (AP2) was assisting AV with a transfer during which time AP2 threatened physical body hard to AV. AP2's actions are considered Verbal/emotional abuse. The facility failed to protect AV from abuse which is a violation of Oregon Administrative Rules.
