Findings
The Alleged Victim (AV) had a history of wandering and exit seeking. On January 20, 2021, AV eloped from the facility campus by turning over a potted plant and climbing the fence. At some point during his/her elopement, AV fell and sustained an injury to his/her head. The facility failed to provide a safe environment for AV. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
