Findings
Witness 1 (W1) has a known history of aggressive behavior and is care planned with interventions. On or about January 20, 2021, W1 was administered his/her medications which agitated W1 as evident by yelling. Alleged Victim (AV) approached W1 and grabbed his/her arm and asked W1 to be quiet. W1 hit AV in the face causing bruising. The facility failed to follow the care plan regarding response to W1's aggressive behavior, which is a violation of resident rights, is neglect of care and constitutes abuse.
