Findings
The Alleged Victim (AV) is care planned to have stand by assist while ambulating. On or about January 21, 2021, AV was using his/her walker alone, going back to his/her room. AV fell just inside his/her doorway to the room, and sustained a goose egg to his/her temple. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
