Findings
The facility failed to provide a safe medication administration system for Alleged Victim (AV) which resulted in the facility failing to administer AV's medication as ordered. An investigation determined that on January 21, 2021, January 22, 2021, and January 23, 2021, AV received twice the amount of blood thinner as prescribed. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
