Findings
On or about January 29, 2021, Witness 1 (W1) and Alleged Victim (AV) had a verbal and physical altercation in which W1 punched AV repeatedly in the chin causing AV injury. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
