Findings
The facility failed to provide proper supervision and services to Alleged Victim (AV) which resulted in AV suffering multiple falls which resulted in risk of serious harm. AV suffered thirteen (13) falls between November 26, 2020 and January 28, 2021. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
