Findings
On or about February 1, 2021, Alleged Perpetrator #2 (AP2) was assisting the Alleged Victim (AV) and during this time, was not aware of AV's diagnosis. AP2's actions exacerbated AV's condition, causing AV unreasonable discomfort. The facility failed to provide service plans to AP2 so he/she would understand AV's condition. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
