Findings
The facility failed to provide basic care and services to Alleged Victim (AV) that were essential to AV's health and well-being which resulted in AV having significant weight loss in a short period of time. AV's care plan did not specifically address AV's personal nutritional needs and failed to provide adequate interventions to ensure AV was getting adequate meal intake. The facility's failure to properly plan care for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
