Findings
On or about February 4, 2021, Alleged Perpetrator 2 (AP2) screamed at Alleged Victim (AV) and used a derogatory term regarding the situation when AP2 was acting in the role of caregiver in the facility where AV was a resident. AP2's actions is considered verbal/emotional abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
