Findings
The facility failed to follow Witness #2's care plan to keep him/her in line of sight due to altercations with other residents. The failure resulted in a physical altercation with AV, causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
