- The facility failed to provide basic care and services necessary to maintain Alleged Victim's (AV) health and safety which resulted in AV's health declining after he/she returned to the facility after being in the hospital with a COVID-19 diagnosis. An investigation determined that facility failed to provide oversight and monitoring of AV's change in condition which resulted in decline in mental status, development of pressure sores, and significant weight loss. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
