Findings
The facility failed to appropriately care plan for the Alleged Victim (AV) regarding his/her risk of falls. AV was found on the floor from a fall, causing a goose egg to his/her forehead. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
