Findings
The Alleged Perpetrator 2 (AP2) caused unreasonable discomfort to the Alleged Victim (AV) by non-accidental means. On or about February 11, 2021, AP2 pushed AV, in result AV was transported to the hospital. The facility failed to protect AV from physical abuse which is a violation of Oregon Administrative Rules.
