Findings
On or about February 5, 2021, the facility failed to assure Alleged Victim (AV) received his/her prescribed oxygen required for his/her medical condition which resulted in AV suffering difficulty in breathing and was transported to the hospital for treatment. The facility's failure to provide AV with timely medical treatment is a violation of resident rights, is considered neglect of care and constitutes abuse.
