Findings
On or about February 17, 2021, the Alleged Victim (AV) and Witness #1 (W1) were involved in a resident to resident altercation where W1 hit AV in the arm. Neither resident was injured in the altercation, however, AV was placed at risk for harm. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
