Findings
On or about February 23, 2021, the Alleged Victim (AV) was without his/her power chair for approximately two weeks. During that time, AV was mostly in his/her bed, unable to move about the facility, except for the use of a manual wheelchair, which was too small for him/her. During the time AV was mostly bed bound, AV developed ulcers on his/her behind, causing unreasonable discomfort. AV was not checked on regularly while the facility call system was down, and was found in his/her bed soiled from not being able to get staff to take him/her to the restroom. On February 17, 2021, the ulcers on AV's behind were red, swollen and draining fluid. The facility failed to provide service to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
