Findings
On or about February 19, 2021, the Alleged Victim (AV) and Witness #1 (W1) were engaged in a resident to resident altercation resulting in AV receiving a bloody lip. AV and W1 had previous altercations and were care planned to be kept apart from each other. The facility's failures are a violation of resident rights, are considered neglect of care and constitute abuse.
